After you've created the foundation, seen here, the balloon hat can be embellished however you desire.
    Unicorn Balloon Hat

    Project designed by Addi Somekh.

    Materials:

    bag of long balloons
    pump

    1. Hold the beginnings of the two long balloons together — and with your other hand, pinch two ballies and twist them together. Twist around three or four times.

    2. Begin the swirl — firm and slow.

    3. As you reach the very end of the swirly, pinch two ballies with one hand and twist them together (figure A).

    4. Take the head base made earlier and attach the swirly to it. This will make the unicorn hat.

    5. Measure and pinch the middle back of the headband.

    6. Connect the bottom of the swirly (with the ties) to the back, middle of the headband.

    7. Take the upright limb of the base and stick it through the swerly — about one third of the way up from the bottom (figure B).

    8. Add a curly to the hat by twisting the balloon.

    9. Add a dingy by drooping the balloon onto the hat.
